More than 2,300 healthcare providers and organizations in the greater Fort Collins area are listed on HealthInfoSource.com. Providers are categorized by type of healthcare:

1. Medical Care
2. Dental Care
3. Other Health Services
4. Mental Health and Substance Abuse Services (MHSA) 
5. Complementary and Alternative Medicine (CAM)

If you're not sure what category you need, see the care index below.

Can't find what you're looking for? Here is a list of common problems:

1. Spelling or typing error in keywords.
2. Practice or agency not located in the Fort Collins area.
3. Searching under the wrong type of care. See the care index below to make sure you are searching in the right category.
4. Search parameters are too narrow. Try using fewer keywords or specifying fewer criteria.
5. Practice or agency isn't listed on the web site. If you have tried everything and still can't find the info you need, please send an email to pklein@healthdistrict.org to suggest a resource.

There are two ways to search the database for the healthcare you need. If you know the name of the agency or provider you're seeking, try a keyword search. If you're starting from scratch, try searching by type of care.

How the Database Works
This site stores three levels of healthcare provider information:

1. Organization
2. Program
3. Provider

Organizations

Healthcare organizations are the top level in the database. All healthcare agencies and organizations in this database will have an organization record, whether a large nonprofit agency or a small one-provider private practice. These records provide all or some of the following information:

1. Name or organization or agency
2. Description
3. Funding (nonprofit or governmental, for-profit)
4. Email and web site
5. Office address and phone
6. Disability access
7. Languages spoken

Programs

Each organization can specify zero or more programs/services below it. Examples of programs and services include Healthy Weighs (a program of Health District of Northern Larimer County) and Family Support Services (offered by Foothills Gateway). To ensure that smaller private practices come up in searches, most private practices will also have a program record, though some of the information may repeat from the organization record. Program records provide all or some of the following information:

1. Program name (if a private practice this will be the same as the organization name)
2. Description
3. Year founded
4. General hours of operation
5. Type of care
6. Ages served
7. Payment/fee structure
8. Eligibility
9. Childcare availability
10. Population served
11. Program duration

Providers

Programs may specify zero or more individual providers. Provider records include all or some of the following information:

1. First and last name
2. Gender
3. First year of practice
4. Description
5. Education
6. Hospital privileges (mainly for physicians)
